Domestic surgical robots are used for clinical use for the first time

Release date: 2014-04-09

The Third Xiangya Hospital of Central South University announced in Changsha that the three domestic robotic operations initiated by the hospital in China were successfully completed in the past week. This is the first time that the surgical robot system independently developed in China has been used in clinical practice, which indicates that China has stepped out of a road of independent innovation of surgical robots and broke the monopoly of foreign surgical robot technology in the world.
According to Zhu Sunhong, deputy director of the Third Xiangya Hospital, three operations were carried out on March 26, March 31 and April 2. The patients were two middle-aged men and one middle-aged woman, one of whom A man was admitted to the hospital for diagnosis of gastric perforation with diffuse peritonitis and the other two for appendicitis. After ethical review and informed consent, the hospital underwent “laparoscopic exploration + surgical robot-assisted gastric perforation repair” and “laparoscopic exploration + surgical robot-assisted appendectomy” under general anesthesia, and the patients remained stable after surgery. Two male patients recovered well.
The surgical robot is essentially an "endoscopic surgical instrument control system", which is generally composed of a doctor's console, a robotic arm system, and a 3D imaging system. During the operation, the doctor observes and operates on the console. The system transmits the doctor's motion signal to the robot arm through the “remote control” and converts it into a synchronous real action of the robot in the patient to complete the surgery.
The surgical robot system used in this study was jointly developed by Tianjin University and Central South University. It adopts a master-slave operation mode and is equipped with a doctor's console and a robotic trolley. In the middle of the doctor's console, two sets of main operating handles and a control panel are installed. The top of the console is integrated with a display box to "see" the real-time stereoscopic scene information of the patient. The robotic trolley consists of three robotic arms with a “hand” at the end that has 3 to 4 degrees of freedom for quick “grab” and “replacement” of surgical instruments.
Wang Shuxin, dean of the School of Mechanical Engineering of Tianjin University, told the reporter that the main hardware, software, materials and system design of the surgical robot are independently developed by China and have independent intellectual property rights. Thanks to the innovative wire drive decoupling design technology and lightweight reconfigurable system design, the robot is lighter and smaller than foreign ones, with lower cost and modular assembly. It is a new type of cavity. The performance of the mirror-assisted surgical robot system is comparable to that of similar foreign equipment.
Reducing the surgical trauma of patients has always been an important goal of modern medicine. In the 1980s, the application of laparoscopy opened the era of “miniac invasive surgery”; the surgical robots born in the early 21st century advanced the surgery to the “third generation”. Compared with the first-generation open thoracotomy and second-generation endoscopic minimally invasive surgery, robotic surgery can make the surgical field clearer, the operation operation more precise and accurate, and help the doctor to complete longer in sitting position. Complex surgery of time.
Experts said that the surgical robot system is suitable for the following surgical operations: one is cholecystectomy, appendectomy and other robotic surgery that has no effect on conventional laparoscopic surgery; the second is hepatectomy, complex biliary reconstruction, etc. can significantly improve the effect of laparoscopic surgery Robotic surgery; the third is the visceral aneurysm resection and anastomosis, intra-abdominal lymph node dissection, and other operations that are currently difficult to perform accurately under laparoscopy. Although the three operations performed this time are relatively simple, the ability to perform more complex operations is already technically available.
It is understood that in developed countries, the surgical robot system represented by the American "Da Vinci" has been widely used in the fields of general surgery, urology, cardiothoracic surgery, etc., and has achieved great success. In recent years, a number of well-known domestic hospitals such as the People's Liberation Army General Hospital and Peking Union Medical College Hospital have begun to introduce this technology from abroad. The successful development of the domestic robotic surgery has made China one of the few countries to master the technology from equipment research and development to clinical research. The long-term dependence on high-end medical equipment such as surgical robots is expected to change significantly.
